Obtain information such as general demographics, state of Ohio license status, and last annual Medicare and/or Medicaid certification survey date.
Health Care Provider Report and Information Extract:  Generate, print, and download reports regarding health care providers licensed and/or certified by the Ohio Department of Health, Office of Health Assurance and Licensing. Output such as a Facility List report can be viewed and extracted as Adobe PDF, Microsoft Word, Microsoft Excel, Comma Delimited Text, and XML formats for further customization.
Health Care Provider Online Business Processing - Enhanced Information Dissemination & Collection (EIDC):  Secure access for long-term care facilities (Nursing Homes, Residential Care Facilities and Intermediate Care Facilities for Individuals with Intellectual Disabilities), non long-term care facilities (Hospice, Health Care Facilities, Home Health Agencies, etc.), Nurse Aide Training Competency (NATCEP) entities to complete business processes online. These processes include, but are not limited to, renewing licenses/programs, submitting payments, updating information (e.g., nurse aide employment roster information, facility demographics, etc.), registering for events/training, viewing/receiving bulletin board notifications, and submitting facility self-reported incidents of abuse, neglect and misappropriation. Environmental Abatement & Licensing System (EALS) – Lead Professionals:  The Ohio Department of Health has developed a secure on-line application for lead licensed individuals to assist in complying with the requirements of the Ohio Administrative Code. Lead contractors can submit and update prior notifications. Lead inspectors, risk assessors and clearance technicians can submit monthly summaries.
